From 27 April 2026, a new Annex 1 (Załącznik nr 1) to the application for a temporary residence and work permit and a Blue Card will come into force. The changes are related to the launch of the MOS system and the digitisation of residence procedures.
The new Annex 1 (załącznik nr. 1) should be prepared with particular care. It is on the basis of this document that the immigration office assesses the terms and conditions of the foreigners employment.
Importantly, the practices of immigration offices are not yet standardised.
➡ Łódzki Urząd Wojewódzki requires forms dated 1 December 2025 and the latest forms (after 27 April 2026),
➡ Dolnośląski Urząd Wojewódzki requires a new form,
➡ Zachodniopomorski Urząd Wojewódzki also uses a new form,
⚠️however, Mazowiecki Urząd Wojewódzki still requires corrections to be made on the forms in force on the date of application.
The use of an incorrect Annex 1 form may result in a request to rectify formal deficiencies, and consequently prolong the entire procedure.
📌 During the current interim period, it is particularly important to monitor announcements from different immigration offices, as the practice of using the new forms is still evolving.
